Children and Young People with SEND may need support from a range of health professionals, they may work for lots of different organisations.

If you are not sure about the role of a health professional or who they work for, please ask them or another health professional you feel comfortable talking to.

You might hear people talking about primary care, secondary care, and tertiary care:

• Primary care is often the first point of contact for people in need of healthcare, usually provided by professionals such as GPs, dentists and pharmacists.
• Secondary care, which is sometimes referred to as 'hospital and community care', can either be planned (elective) care such as an operation for an ongoing condition, or urgent and emergency care such as treatment for a broken leg.
• Tertiary care refers to highly specialised treatment that only small numbers of people will have such as transplants and mental health hospitals. Leeds Teaching Hospital is a tertiary centre for a number of specialist treatments.
