At this stage young people may start to think about where they would like to live. They may decide they would like to continue to live with their family and become more independent at home, or they may decide they would like the chance to try and live independently in the community. This page provides advice, guidance and links to sources of support for young people to make choices about living as independently as possible.

Assessing your social care needs

Adult social care help adults who need extra support to live well.

Advice and needs assessments are free, but our care and support services usually cost money. If you qualify, you may get help to pay for the care and support you get.

After your needs assessment, we can refer you to see if you qualify for help to pay. Your care can start even if you have not had a financial assessment yet.

Support for living independently

There are a number of organisations in Leeds that can support you with skills for in the home.

People Matters (11+ years)

We can assist with lots of things like cooking, shopping, benefit queries, banking, light household duties, making friends, exercise, and healthy living plus joining local groups and activities. Now we are also CQC registered. This means if you have more complex support needs such as personal care, we can support this type of need too.

Care and Repair (all ages)

We help our clients to continue living independently in their own homes by making them warm, safe and secure, and work with our partner organisations to ensure our clients can access all the support they need. We provide minor adaptations to prevent falls, larger adaptations such as accessible showers, small repairs to remove hazards and ensure the heating is working effectively, advice and information on a range of issues such as welfare benefits.

LiveYou (16+ years)

LiveYou supports young adults with physical disabilities, creating custom live-in support packages which maximise choice & control. LiveYou is user-led, supporting clients to: acquire funding, recruit PAs and manage their support. Fully independent, LiveYou upholds users’ rights and maximises the funding they receive.

GIPSIL – Gipton Independent Housing Support (all ages)

GIPSIL provides housing and support across Leeds to enable people to live in their own homes. We provide specific services to young people, care leavers and adults who live with complex support needs.

Leeds Centre for Independent Living (all ages)

Leeds CIL provides a variety of support services for disabled and older people in the Leeds area who want to take control of any help they need with care and daily living tasks.

The Independent Living Teams support disabled people of all ages to take control of any help they need with personal care, daily living tasks and getting out and about in their community. This includes supporting people to recruit, manage and pay their own support workers, usually called Personal Assistants (PAs). Employing PAs enables our customers to choose who they want to provide their support and how and when the support is provided, to suit their and their families' lifestyles

Home from Home Care (18+ years)

Home From Home Care provides specialist residential care services for adults (18yrs+) with learning disabilities, autism and complex physical & mental health.

Combining a family perspective with an uncompromising approach, we design exceptional environments and bespoke care and support packages around each individual’s needs. setting goals for an ordinary life – ‘The World On My Terms’.

We design bespoke support packages and tailor environments to suit each individual’s needs. We use creative solutions and technology to promote independence, support individuals to lead fulfilled and active lives through meaningful activities and build on the life skills that young people have been taught at school and college.

Services to improve your home

Adapting your home, Leeds City Council

If you’re finding it difficult to move around your home, including using the bathroom or kitchen, you may be eligible for a grant.

You can use the grant to help pay for adaptions to make it easier to move around and access your home. This can include:

  • installing ramps and widening doors
  • improving access to rooms with stair lifts or downstairs bathrooms
  • building wet floors or walk in showers

If you would like to apply for the grant, you will need to contact the Social Care team. They will arrange an assessment, which includes an Occupational Therapist visit to your home.
